Management of Trachea Dilatation Patients Induced by Prolonged Overinflated Tracheostomy Tube Cuff Using a Portable Ventilator

Abstract
Tracheostomy is applied to patients who require long-term tracheal intubation. Unfortunately, this can result in tracheal dilatation due to gradual expansion from the tracheostomy cuff. Enlargement of the trachea can lead to insufficient ventilation from the leakage through the widened space. However, to this date, there is no clear solution. Here we report an 84-year old male patient who presented with dilated trachea due to improper use of the tracheostomy cuff pressure. The patient¡¯s caregiver arbitrarily infused air into the cuff, thereby widening the trachea imperceptibly. Consequently, the patient presented with dyspnea and desaturation even when a portable ventilator was applied. The problem was resolved when we implemented an adjustable tube, which allowed for the revision of the cuff position to avoid the placement within the dilated space. Here we propose that the proper tracheostomy tube cuff management and ventilatory care can be used to successfully manage tracheal dilatation.
